What is rhinoplasty?

Rhinoplasty — or “nose job” — refers to surgery that changes the shape of the nose and includes several different cosmetic procedures that serve both aesthetic and reconstructive purposes.

This means that rhinoplasty can not only enhance your cosmetic appearance, but it can also improve the functionality of your nose.

Dr. Bassiri and our team can often combine rhinoplasty with surgeries such as septoplasty, nasal valve repair, and turbinate reduction to improve breathing through the nose. Together, these can sometimes be referred to as “septorhinoplasty.”

What is the difference between open rhinoplasty & closed rhinoplasty?

Dr. Michael Bassiri-Tehrani understands the importance of feeling confident and happy with your appearance.

But if you’re like many of our other patients, your nose can be a source of insecurity. You may feel self-conscious if your nose is too large or too small or has an unappealing shape.

Depending on your aesthetic goals and anatomical needs, you may require either an open or closed rhinoplasty — which simply just refers to how your nose is accessed during surgery.

  • Closed rhinoplasty: This procedure is performed with incisions hidden inside the nostrils without any external incisions. In the correct patient, this is an ideal option to make discreet changes. This technique treats the dorsum or bridge of the nose.
  • Open rhinoplasty: Revision surgery or nasal tip work is often performed with an “open approach.” Open rhinoplasty uses the same incisions as a closed rhinoplasty but adds small incisions at the base of the nose, or collumella. This collumellar incision heals very well but allows for much better access to the nasal tip and midvault or valve region of the nose. This enables us to make more precise changes as well as correct complicated deviations and breathing problems should they exist.

What happens during your procedure

Rhinoplasty is typically performed under general anesthesia, so you’ll be asleep throughout the procedure.

Typically, the surgery can take up to four hours to complete. During this time, Dr. Michael Bassiri-Tehrani will make incisions either inside your nostrils or across the columella — the tissue between your nostrils — so that they can access the underlying bone and cartilage.

Your surgeon will then reshape and sculpt the bone and cartilage to create the desired shape and size. Once the reshaping is complete, the incisions will be closed using dissolvable sutures for a more comfortable, worry-free recovery.

What your recovery looks like

After the procedure, you’ll of course need to rest and recover for a period of time determined by extent of your nose’s restructuring. You may experience some swelling, bruising, and discomfort, but you can manage this symptoms with medications, and they usually subside within a week or two.

As with most surgical procedures, you’ll need to avoid strenuous activities and heavy lifting for a few weeks to ensure proper healing. Dr. Michael Bassiri-Tehrai will provide you with detailed instructions for post-operative care to help ensure your recovery is as smooth and comfortable as possible.

What to expect during recovery for rhinoplasty

Recovery for both open and closed rhinoplasty is very similar. The surgery typically lasts about three hours and is performed with general anesthesia. After a short recovery room visit, patients go home the same day.

Pain is controlled with mild narcotics the first two to three days, after which most patients switch to Tylenol and mainly experience congestion.

Patients don’t receive traditional nasal packing, a known discomfort from an older time. Instead, some patients may have silastic sheeting or internal splints placed temporarily if additional support is needed.

At one week, all sutures, splints, and casts are removed. The nose is cleaned and most of the congestion is gone. Bruising usually takes about seven to ten days to resolve. Most of the major swelling takes about two weeks. By five to six weeks after surgery, most of the swelling is gone.
